To become a food vendor please fill out the following application:

 In order to participate in the Cultural Survival Bazaars you must have:

1. Liability insurance
2. Active food license/ Safeserve certificate
3. Temporary Food Permit from each town you will be vending (enclosed and available on our website)
4. If you are interested in being a food vendor at any of our Boston, Massachusetts events please fill out the additional form (tempfoodserviceapp.pdf)
